[CEUD-ICT] Accessible Online Survey

Hi Declan,
something we've been looking at here that may be a fit for a "simple"
survey is Wufoo [ 1].  It's really a form builder but for shorter single
page surveys it seems fine.  We are currently trialing it and Alan has
identified a number of smaller accessibility issues.

Its free offering allows 3 forms with 10 fields per form and 100
submissions per month.

Hello All,
  I need to put together a simple accessible online survey.
Would anyone know of a free online system I could use or alternatively
a technology that you'd recommend in order to accomplish this?

Any guidance would be much appreciated
